Ive been really lucky when it comes to breakouts. Ive written about them, Ive had a few pop up from time to time, and Ive recommended products to zap them, but Ive never suffered from the frustrating woes of teenage or adult acne. That being said, something recently changed. Perhaps its because Ive recently ascended into my late-20s , but now, whenever I break out, no matter how small or inconsequential the actual pimple is, it leaves behind a red mark long after it clears up. It is so annoying. How are acne scars and discoloration problems only just starting to plague me now? Arent these years supposed to be zit-free?

Thats why I spoke with experts to get their opinions on why this is happening and advice for how to make it stop. Like, immediately.

How Do You Treat Boxcar Acne Scars

This type of scar can be treated with subsicion, which involves a dermatologist or PA using a sharp needle underneath the surface of your skin to break up that tough scar tissue. Punch excisions are another option, which means a dermatologist will remove the indented area of the scar and then use a suture to pull the edges of the scar together, closing the gap. Collagen-boosting laser treatments are also used in conjunction with these treatments to encourage the skin to produce collagen and naturally re-plump itself. Skincare professionals can also use fillers, such as Bellafill, or fat transfers to fill in boxcar scars.

Not just here to make you look like a contestant on Love Island, fillers can be used to help an array of skin problems, one of them being acne scars. A tiny amount can temporarily plump and smooth over any skin craters left behind from acne.

‘Dermal fillers are very good for indented scars’, says Dr. Anjali Mahto, Consultant Dermatologist and British Skin Foundation Spokesperson. ‘However, depending on the type of filler used, the treatment will need to be repeated at regular intervals – most probably every six months – to maintain the results.’

Your skin’s initial reaction may be to bruise, swell up and go red but be patient and wait for the magic to happen. As acne scarring treatments go, this one isnt the priciest and is relatively non invasive and pain free. Hooray!

Fillers A substance such as collagen, hyaluronic acid, or fat can be used to “fill out” certain types of acne scars. Since fillers are eventually absorbed into your skin, youll have to repeat filler injections, usually every few months, depending on the substance used.

Punch excisions This type of skin surgery removes acne scars by individually excising, or cutting out, the scar. The hole left by the incision can be repaired with stitches or a skin graft. A skin graft uses a small piece of normal skin to fill in an area of scarred skin. Bruising can occur for one to two weeks after the procedure.

Microneedling Also called collagen induction therapy, this procedure involves pricking the skin repeatedly with tiny needles to stimulate the growth of new collagen. This can reduce the appearance of scars. Microneedling can improve the appearance of wrinkles, stretch marks, and fine lines. Skin redness may last a few days, according to the AAD.

Other treatments that a dermatologist may use to treat raised acne scars follow.

InjectionsYour dermatologist may recommend injecting medicine directly into the scars. This can soften and flatten raised, thick scars.

Getting the best results often requires repeat visits. These injections are usually given once every few weeks. How often you will need to return for treatment depends on the scar and many other considerations. You may need to return once every two to six weeks for a while.

Many patients receive injections of corticosteroids. A chemotherapy medicine known as fluorouracil can also be effective in treating raised acne scars. Some scars respond best when injections of both 5-FU and corticosteroids are used.

Interferon, which is also used to treat cancer, can be effective. Our bodies naturally produce interferon.

If the scar does not respond after you receive the 4th injection, acne scar surgery may be recommended.

    Best for: Painful, raised scars

Acne scar surgeryDermatologists perform acne scar surgery to reduce raised acne scars. This surgery can be performed in a dermatologists office.

To obtain the best results, acne scar surgery is often followed by another treatment.

Acne scar surgery, followed by radiationStudies show that radiation can prevent raised scars from returning after acne scar surgery. Due to the potential for radiation treatments to cause problems years or decades later, some doctors do not recommend radiation treatment.

    Typesof Acne Scars

The nomenclature for categorizing types of acne scars has not been entirely settled in the literature. For our purposes, it is useful to recognize three main categories of depressed acne scarsicepick scars , boxcar scars , and rolling atrophic scars . Atrophic acne scars are the most common and are likely caused by inflammatory processes leading to the enzymatic degradation of the collagen fibers and subcutaneous fat. There may be an inflammatory component to some acne scars. Clinicians may also encounter scars with more than one physical characteristic, such as pigmentation or erythema, in addition to being atrophic or hypertrophic. These may be termed hybrid scars. Some acne scars are elevated above the surface of the surrounding skin and may be hypertrophic, but not quite keloidal. Other characteristics that can help define acne scars include textural irregularities, depression, aberrant pigmentation, erythema, vascular dilatation, unclear or irregular borders, and papules.

There are several ways to classify acne scars. In one method, acne scars may be graded on a four-point scale with grade 1 describing macular scars and 4 being severe disease including scarring that is not flattened when the skin is manually stretched. The treatment of acne scars must be customized to meet the needs of the patient , the patients goals, and the patients lifestyle . See for a short summary of main types of acne scars and treatment considerations.

    Microneedling

There are many treatments out there to help fade the various forms of acne scarring including lasers and microdermabrasion but Marshalls preferred treatment is microneedling. Performed by a professional, microneedling is probably the best for scarring, and also helps reduce the signs of ageing,” she explains. The process involves using tiny needles to create micro-punctures in the skin, which kick-starts cellular renewal, resulting in greater collagen and elastin production.

Ayodele agrees, adding that “the earlier this is done the better, as it’s much more challenging to treat old scars.”

    Peels

If your scars aren’t severe, a chemical peel could be a wise option. “For less obvious scarring, having a clinical treatment with a low-pH acid will make a big difference,” says Marshall.

What’s more, at-home peels have come a long way in recent years, and there are now plenty of excellent options that work on renewing the skin surface, reducing the depth and intensity of acne scarring. “To treat acne scars at home, use a product that offers gentle chemical exfoliation,” suggests Dr Gross. Opt for a pre-soaked pad for ease of use, or try a toner-style resurfacing liquid.

    Acne Scar Treatment No: Retinol

Also known as vitamin A, retinol goes hand in hand with anti-ageing treatments but can also be used to unclog pores and fade scars. Its a more gradual treatment but is popular for leaving skin looking brighter and smoother.

Associate Professor of Clinical Dermatology at USC, Keck School of Medicine, Dr Nada Elbuluk explains the benefits: ‘Many darker skin patients who develop dark spots also known as hyperpigmentation, may refer to them as “scars”. For others, acne scars can refer to textural changes in the skin such as indentations and uneven skin texture. If it’s the former, then we often treat with products such as retinoids which can help with evening out skin tone and treating acne. If it’s more of a textural change, retinoids can also help but typically a resurfacing treatment is also needed’.

Avoid if you have rosacea – it can upset and irritate.

Do trybakuchiol – it’s a natural alternative rich in vitamin C and E that works to provide the same brightening results.

    Why Do Acne Scars Appear

These ones are the result of excessive bacteria growth that occurs in the skin follicles after they are blocked with excessive oil and in the change in the formation of keratin so that it triggers an intense inflammatory response.

In the worst case, this intense response of the immune system leads to large ulcerated pockets. That results in the formation of nodules or cysts which cause scars of different stages.

It might seem like the simplest way to get rid of acne scars and we bang on about it a lot but SPF really does solve all your skin troubles. Not only does it protect against the dreaded sun damage but it helps avoid the more long term risk of skin cancer and, in turn, the marks it can leave on your skin.

But SPF is never more crucial than when youre using acids, retinol or extensive treatments that can leave your skin vulnerable. ‘The skin is more vulnerable to damage and pigmentation change following acne treatments such as IPL laser and even using products that contain AHAs and BHAs and pigmentation spots can potentially get worse is SPF is not applied,’ says Dr. Mahto.
