What Not To Do And What Not To Buy
While topical products may help to keep acne-prone skin healthy, Dr. Kluk is keen to stress that serums and skincare alone wont bring severe cystic acne under control. However, she does recommend sticking to a few skincare principles while on your path to clearer skin…
- Never try to pick or squeeze cysts. This can lead to infection and increase the risks of scarring. Yes, we know it’s tempting, but you will not win.
- Dont attempt to scrub or exfoliate the skin. This can increase swelling and redness.
- Use a mild cleanser and moisturiser, and a sunscreen in the morning too. This may be all that is necessary whilst you complete your course of medication.
As Thomas adds, some products may actually contribute to the formation of blockages in the pores, ultimately exasperating your acne. She advises ditching the oils, balms and richer creams, looking instead for products both skincare and makeup marked non-comedogenic, . ‘Also avoid overly drying products,’ she says. ‘Drying out the surface will rarely affect a cyst, and really just causes more redness and irritation.’
How Can I Prevent Cystic Acne
You can lower your risk of getting acne by taking these steps:
- Use a mild foaming facial cleanser, lukewarm water and your fingers to wash your face after you wake up, before going to bed and after exercising or sweating.
- Apply oil-free moisturizer if you feel dry.
- Use noncomedogenic makeup and facial products.
- Dont sleep in makeup.
- Keep your hands away from your face during the day.
- Dont pick at or pop pimples or scabs.
- Wash your hair regularly and keep hair away from your face.
- Occasionally oily environments, such as working at a fast food restaurant, can contribute to acne flares.
- Diets high in simple sugars and dairy, also called a high glycemic index diet, may contribute to flares as well.
What Role Do Hormones And Endometriosis Play In The Development Of Cystic Acne
The short answer? Usually, a pretty significant one for hormones, and about 1% of patients who suffer from endometriosis also battle cystic acne.
To get more scientific, Dr. Paviol explains it this way, Typically androgens make acne worse and estrogens make acne better. Women are more prone to develop acne later in life as estrogen levels lower with age and women lose some of the protective effects of estrogen over time. Because women with endometriosis often experience hormonal imbalance, they often experience hormonal acne.
Dr. Emily McLean of Novant Health Dermatology SouthPark adds to this by saying, Hormones are known to regulate sebum production, and having an excess of sebum can lead to acne. There are studies that show that patients with severe acne during the teenage years may have a higher risk of endometriosis.
According to one study published in Human Reproduction, 0.5% of patients suffer from endometriosis and cystic acne, while 0.4% of patients have cystic acne but do not have endometriosis. The study concludes that endometriosis is positively associated with severe teenage acne, but the 0.38 vs. 0.49% is too low to use it as a diagnostic criteria.

Lifestyle And Diet Changes You Can Make To Improve Cystic Acne
When it comes to cystic acne, there are several available medical treatment options as well as lifestyle changes you can make to support your treatment plan.
Acne occurs when a pore gets clogged with dead skin cells, trapping normal bacteria in the pore and causing it to multiply. When this occurs, your immune system is triggered and reacts with redness and swelling that eventually results in a pimple.
Cystic acne, however, is more than just your typical pimples.
Cystic acne is caused when inflammation goes deep into the skin, creating a tender red bump that is often filled with pus. These painful breakouts can last for years, and when these cysts burst, they can lead to more inflammation and even permanent scarring.

Why You Shouldnt Try To Treat Cystic Acne On Your Own
Instead of taking the drugstore route to battle cystic acne, Dr. Braue strongly suggests starting with a dermatologist. Over-the-counter products can work when combined with a comprehensive treatment plan, but he says that a dermatologist can step in way before scarring or skin pigmentation problems set in.
If you are living with true cystic acne, get a good dermatologist and dont mess around with over-the-counter products. Now, there are certain over-the-counter solutions that I do recommend to patients in addition to their treatment plans, but the problem with messing around with cystic acne is that it can lead to scarring and dyspigmentation most of the time.
Dr. Braue adds that this is quite common with skin of color. People with more melanin in their skin tend to experience more hyperpigmentation or discoloration because of the inflammation thats associated with cystic acne.
Apply A Hydrocolloid Acne Patch
This is the #1 public enemy of budding cystic acne. For the individual giant cyst that starts to form, I recommend acne patches which can draw fluid from and reduce the swelling of the cyst, says Turner. The patches are often made out of hydrocolloid materials that are super absorbent to help flatten the cyst. In addition, some of these acne patches contain active ingredients such as salicylic acid or tea tree oil that treat the pimple while concealing it.
Most of these patches require six to eight hours of wear, in order to get the best benefits. Dont sell yourself short with a measly two-hour application, either. Wash your skin, gently pat dry, then apply the patch and leave it on overnight, or all day . It can be the difference between that thing erupting on your face, or retreating back into your body. It may linger for a few more days, but youll note that its not increasing in size or fury.

Resist The Urge To Pop Your Pimple
DONT YOU DARE.
Seriously, though. Popping pimples is risky and ill-advised at the best of times, but cystic breakouts are on a whole different level.
Because cysts form so deep within the layers of your skin, they are nearly impossible to drain by squeezing. In fact, one of the hallmark traits of a cyst is a thick lining that makes it even more resistant to your pimple-pinching efforts.
Attempting to pop a cyst is much more likely to result in scarring than relief, your poor epidermis ravaged by all the fruitless poking and prodding. Not to mention the possibility of bursting the cyst beneath your skin, letting all that infected grossness spread and multiply.
Can Tea Tree Oil Help Cystic Acne
If you have mild acne, there is data out there that shows that tea tree oil can be helpful. With cystic acne, when someone is looking to treat it naturally, I always mention that just because something is natural doesnt mean that its necessarily good for you. With tea tree oil, some people are allergic to limonene, a compound thats found in it. This allergy can cause contact dermatitis in addition to their acne. So again, if someone has true cystic acne, they need to see a dermatologist or their healthcare provider first.
How A Dermatologist Might Treat Cystic Acne
Since were all different, there isnt a one size fits all solution for cystic acne. When you talk to a dermatologist, you can work together to develop a treatment plan that works for you.
Dr. Braue says that isotretinoin is still widely used to manage cystic acne and recent studies suggest that there isnt a direct link between it and an increase in depression. If you opt to go this route, he usually recommends just cleansing the skin with something very mild and not an acne wash.
Accutane For Cystic Acne
If there is no response to oral antibiotics or spironolactone , the most powerful medication for cystic acne is isotretinoin, or Accutane. It is a powerful derivative of Vitamin A in oral form. Over 80% of patients see improvement within six months of beginning isotretinoin therapy. However, there are several side effects that can occur, such as nosebleeds, headaches, joint pain, and new or worsening mood disorders. Although rare, if you begin to experience any new or worsening depression you must notify your dermatologist immediately. The most severe side effect of isotretinoin is severe birth defects in pregnant women. A mandatory prescribing and compliance system called iPledge is necessary for the use of isotretinoin. You will be required to be registered into the system and meet the requirements of program to receive the medication. Your dermatologist must be enrolled into this program to be able to prescribe it to you.

Effects Of Cystic Acne
Cystic acne increases the likelihood of developing scars. This scarring is more likely because these breakouts damage and destroy healthy skin tissue. People should never try to pop or pick at their skin. These actions can damage the skin even further and worsen acne and scarring.
People affected by cystic acne often avoid social situations and feel self-conscious about their appearance. Fortunately, the right treatment can help limit scarring and reduce or eliminate symptoms so patients can feel better about their skin.

Can Food Or Other Things Cause Cystic Acne
According to Dr. Braue, the foods that are often associated with acne havent been scientifically linked to cystic acne.
Sugar, chocolate and fast food always get a bad rap, but I havent seen any great scientific data that links them to cystic acne. So, I cant say that if you eat fast food three times a week or eat too much chocolate, your acne is going to be much worse. There isnt enough proof that these foods are direct causes.
However, if you notice that your acne flares up when you eat certain foods, your healthcare provider or dermatologist might encourage you to avoid them.
The one thing that Dr. Braue has noticed is a connection between acne and skim milk.
From the available data that we have, there seems to be a link between the severity of acne and the ingestion of skim milk. I do always ask patients if theyre drinking a lot of skim milk or if theyre taking whey protein. If they are, I counsel them on finding an alternative.

Treatments For Cystic Acne
There are multiple treatments available for people who have cystic acne. Talk to your dermatologist to figure out whats causing the acne and decide which solution is right for you.
Several prescriptions are available for treating cystic acne, including:
- Isotretinoin : Isotretinoin is an oral medication. Its proven to be one of the most effective cystic acne treatments. A dermatologist usually oversees the course of this medication. A treatment course can last 16-20 weeks.
- Hormonal birth control: For people who menstruate, birth control can be an effective cystic acne treatment. It works by balancing your hormones, but it may take up to six months to start seeing results.
- Antibiotics: Antibiotics are often used as a short-term solution to treat inflammation caused by cystic acne.
- Retinoid creams: Retinoids are derivatives of vitamin A. Applying a retinoid cream to your skin helps speed up the skins cell turnover. Skin that regenerates quickly also heals quickly.
There are things you can do at home to help your cystic acne treatment be effective, including:
- Cyst drainage: This is when the doctor makes a small incision and drains the cyst. You should not attempt to do this at home.
- Corticosteroids: The dermatologist may recommend corticosteroid injections. They are injected directly into the cysts by the doctor.
