Over-the-counter products can work when combined with a comprehensive treatment plan, but he says that a dermatologist can step in way before scarring or skin pigmentation problems set in.

Whats an acne cyst?
Acne cysts are more than your average pimple: theyre deeper, more swollen, and they hurt, too! They usually look like a raised red bump without a whitehead or pustule at the surface. Sometimes they stay under the skins surface and dont come to a head .
Why do they hurt so much?!
All that inflammation creates pressure under the skins surface and on the nerves around it, which is why touching it will cause a painful throbbing sensation.
Can I pop it?
Nope. Picking, squeezing, or popping a cystic pimple is the LAST thing you want to do! First of all, it wont workits too deep beneath the skin. Secondly, messing with it only worsens the inflammation and the pain.
Why me?!
When the opening of a pore gets plugged by dead skin cells, the cells start to accumulate inside the pore instead of being shed in the skins usual renewal process.
Sebum builds up inside the pores, too. Mixed with the dead skin cells, sebum turns into sludge that P. acnes bacteria likes to hang out in.
Trapped under the surface of the skin with nowhere to go, that sludge spills out into the skin tissue around the pore.

And of course if you have any pimple that doesnt heal after a few weeks or months, its important to get it checked out because that could be a sign of skin cancer, Dr. Renati says.
Sadly, you are very unlikely to see improvement of nodulo-cystic acne with over-the-counter treatments. Usually topical medicines arent effective in treating these lesions because they are so deep and topical medicines cant penetrate that far, Dr. Khorasani says, noting that cystic acne treatment is typically more of a tailored, multistep approach from a derm.
While youre waiting for your dermatologist appointment, avoid the temptation to pop or pick a cyst or nodule youre probably not going to be able to actually pop something that deep, but you are likely to develop scars from these pimples. So especially with these types of pimples, resist the urge to mess with them.
Depending on your exact situation, your dermatologist might also suggest blue-light therapy, which works by killing bacteria within the pores, Dr. Engelman says. What Products Am I Using
The first one is La Roche Posay Effaclar Duo. I found this very helpful in reducing the appearance of cystic acne. The product contains benzoyl peroxide which really great for fighting cystic acne, because benzoyl peroxide can kills the bacteria that caused the inflammation. The second product is chemical exfoliator. Im using AHA and BHA interchangeably. Salicylic acid is really great for treating cystic acne, salicylic acid can penetrate deep down into the pores, so it could target the dead skin cells that trapped inside the pores .
