Whats The Life Cycle Of A Pimple
Acne starts as microscopic bumps called microcomedones that cant be seen with the naked eye. Oil glands within the skin are stimulated by hormones and enlarge to become overactive and pump out excess oil. Many infected blemishes come from preexisting whiteheads or pimples that are already present in the area within the pores. Bacteria can get stuck behind the blockage and multiply as the skin begins to become inflamed with bacteria, inflammatory cells, and oil.

This is all important because research has shown there are androgen receptors in the base of the oil gland and also in the cells that line the pores! If there are high levels of testosterone floating in the body, they are highly capable of binding to these receptors.
To put it simply, when these hormones attach to the oil glands in the skin, they trigger the gland to produce more oil as well as feed the bacteria. Then, an inflamed acne lesion is well on its way to the surface of your skin.
The key takeaway is that by lowering androgen and testosterone levels in the body, the skin will not produce as much acne or not produce ANY at all.
So how does Estrogen play into all this?
Estrogen and testosterone frequently act like a teeter totter. As one goes up the other goes down.
Hormonal acne is an elevated androgen/testosterone and low estrogen issue. It is not an elevated estrogen problem. Therefore, taking herbal supplements that block estrogen, such as DIM, will make your acne worse and increase testosterone causing cystic breakouts. I do not recommend DIM for hormonal acne for these reasons.
So it makes sense that if you lower androgen levels, the skin will not produce as much oiland your acne will improve!

What Acne Treatments Are Best For Me
That depends on many factors: your age, whether youâre male or female, how severe your acne is, and how long youâve had it, among others. There are several options available.
For mild to moderate acne, many dermatologists will start with a combination of a topical cream or gel containing either a retinoid or benzoyl peroxide along with a topical antibiotic. For more inflammatory acne, an oral antibiotic may be added. For more significant cases of acne, women may be placed on birth control pills or on the drug spironolactone, a water pill which also blocks male hormones. They may also prescribe the newly approved topical treatment clascoterone which works a lot like spironolactone, only as an ointment.
Severe cases of acne may be treated with the drug isotretinoin, which is very effective. However, side effects and blood abnormalities must be monitored monthly and requires registration with the FDA to obtain a prescription. There are also various types of light or photodynamic therapies available.
Does Adult Hormonal Acne Go Away
While the breakout cycles themselves usually only last two to three weeks, you may be dealing with hormonal acne for years, especially during your 20s and 30s and then again going into menopause, Dr. Hartman warns. But, with the right treatment plan, it is possible to keep hormonal acne breakouts at bay.

Take Oral Acne Medication Or Birth Control
If there are no additional concerns, hormonal acne is relatively easy to treat. We typically start treatment with a birth control pill containing estrogen and progesterone. The FDA has approved several contraceptive medications specifically for acne in women.
Unfortunately, the convenient forms of birth control do not help with hormonal acne. These are only progesterone-based, which can make acne even worse. If a patient is already using one of these devices, well use an alternative treatment rather than ask them to change their birth control method.
Spironolactone is often our first choice for patients not wanting or needing birth control. A low-dosage form of this oral pill is a highly effective method to get rid of hormonal acne.
It blocks the androgen receptors in the skin that are responding to the testosterone by increasing our oil production and making us breakout. Spironolactone essentially blocks the skin from hearing the testosterone say, Hey, make a pimple! Its tolerated well by patients and can be used for long periods of time . Its a highly effective option that often eliminates the need for patients to eventually take Accutane.

How To Treat Hormonal Acne
Everyones skin is different and reacts differently, so it might take some trial and error before you find what works best for you.
Using a face mask weekly in addition to your regular routine is a convenient way to treat the early signs of hormonal acne with simple over-the-counter products, says Dr Tomizawa.
Choose a mask that contains sulfur as it will help with inflammation and reducing oil. Sulfur draws impurities to the surface and helps to unclog blocked pores. It is a keratolytic, which means it can act as a natural exfoliant that helps the new layer of skin underneath regenerate and grow.
Cleansing, treating and moisturising is also an effective solution for treating any form of acne. Ensure that you are using products that are specifically designed for sensitive and oily skin with active ingredients.
Other ingredients you should be looking out for include benzoyl peroxide, and other aforementioned keratolytics such as salicylic acid, glycolic acid and azelaic acid which are all available over the counter.
Many women, however, find benzoyl peroxide can be too drying on the skin so salicylic acid and glycolic acid can be good options to try instead.
Some types of contraceptive pill are often prescribed by doctors as treatments for acne.

What Causes Adult Acne
A number of things cause acne, from allergic reactions to diet and lifestyle. Dr. Thornfeldt says acne may be caused by the skin’s allergy to yeast. If you thought yeast issues were only something that happened, well, down south, the skin can also experience yeast overgrowth, which causes aggravation and breakouts. This may be a result of greasy lotions and oils, naturally oily skin, oral contraceptives, and stress, to name a few factors.
“Diet and lifestyle play a role in everything with the body from arthritis to wrinkles. Lifestyle isnt the cause of adult acne, but it’s an influential factor. Everyone must consider diet, sleep, and exercise, and minimize pro-inflammatory foods in their diet, like sugar and dairy. Gluten is controversial, but sugars and dairy are the biggest factors and people must cut back,” explains NYC-based cosmetic dermatologist Dr. Paul Jarrod Frank.
